Job summary

Western Geriatrics & Neurology is seeking a compassionate Nurse Practitioner or Physician Assistant to join our Mobile Primary Care Team, delivering in-home and community-based care to older adults. You’ll provide comprehensive assessments, develop individualized care plans, manage chronic conditions, and collaborate with multi-disciplinary teams to support whole-person health.

Qualifications

• Active, unencumbered NP or PA license in the practicing state.
• Minimum of 1–2 years of clinical experience (geriatrics or primary care preferred).
• Compassionate, patient-centered approach and strong clinical judgment.
• Valid driver’s license and reliable transportation.

Responsibilities

• Deliver personalized, high-quality care to older adults in homes and senior living settings.
• Conduct comprehensive assessments and develop individualized care plans.
• Manage chronic conditions with skill and empathy.
• Collaborate across clinical disciplines (primary, psych, palliative, neuro) for whole-person care.
• Build trusted relationships with residents, caregivers, and community partners.
• Maintain thorough documentation and consistent communication.
