Qualifications

• Active Nurse Practitioner license in Kentucky (KY)
• Experience in Skilled Nursing Facility (SNF) or long-term care preferred
• Strong clinical assessment and decision-making skills
• Ability to work independently while collaborating with a multidisciplinary team
• Excellent communication and interpersonal skills

Responsibilities

• Conduct comprehensive patient assessments and develop individualized treatment plans
• Diagnose and manage acute and chronic conditions in coordination with physicians
• Educate patients, families, and facility staff on disease management and preventive care
• Maintain accurate and timely documentation in compliance with regulatory guidelines
• Collaborate with interdisciplinary teams to ensure quality patient outcomes
