Job summary

Partners Behavioral Health Management is seeking a Mobile MHSU Care Manager in Gastonia, NC. This full-time role focuses on engaging adults and youths in mental health and substance use services through active care management and treatment planning, with work across community settings.

The position requires a relevant human services degree and licensure, plus experience in care coordination, assessments, and team-based care.

Qualifications

• Bachelor’s degree in a human service field and at least two years of full-time experience with the population served
• Master’s degree in a human service field and one year of full-time experience with the population served
• Current unrestricted licensure and/or provisional licensure as required for NC MHSU roles

Responsibilities

• Provide education, referrals, care management activities surrounding available services and supports including Physical Health, Behavioral Health, I/DD, LTSS, TBI, Pharmacy, Vision, and Dental services/supports
• Link to needed behavioral health and physical health care services and facilitating appropriate connections to primary healthcare services through Community Care of North Carolina, the Health Department, or other community health resources
• Coordinate and link members to benefits
• Complete initial and yearly Care Management Comprehensive Assessment and Care Plan
• Conduct Care Team meetings and ensure participation of treatment team members
• Monitor progress towards goals identified in Care Plan through in-person and collateral contacts
• Identify gaps in needed services and intervene to ensure appropriate care
• Refer member to community resources
• Oversee care transitions between clinical settings
• Maintain accurate tracking of care management activities and outcomes
