Job summary

Powerback Rehabilitation to You is seeking a skilled Occupational Therapist to provide in-home outpatient rehab for older adults across Vernon, Manchester, Coventry, East Hartford and nearby CT communities. The role supports a flexible 30–40 hour work week with a focus on function and independence in the patient\'s home.

You will evaluate and treat in functional environments, guide patient participation, and coordinate with the rehab team to achieve treatment goals.

Qualifications

• Bachelor's degree in an ACOTE-accredited OT program is required.
• Initial registration from the National Board for Certification in Occupational Therapy is required.
• Valid OT license as required by the state of practice and a valid driver's license are required.

Responsibilities

• Provide in-home outpatient occupational therapy services to older adults.
• Supervise OTAs, temporary licensees, students, and support personnel as permitted by licensure.
• Maintain timely documentation of clinical and billing activities and participate in team meetings.
• Design and implement OT plans of care, adapt programs to patient needs, and ensure safety. 
• Collaborate with physicians, caregivers, and rehab team to optimize patient outcomes.
