Job summary

The Community Licensed Practical Nurse (LPN) at PACE Southeast Michigan supports participants as defined in the IDT plan of care. The LPN assists RNs and providers by collecting assessment data and administering medications within scope of practice.

Responsibilities include documenting medications, communicating findings to the medical team, and advocating for participant rights and satisfaction. Must have MI LPN licensure and at least one year with frail or elderly populations.

Qualifications

• Must be a Licensed Practical Nurse with current Michigan licensure.
• Ability to establish and maintain interpersonal and interdisciplinary relationships.
• Assists with nursing research studies through data gathering.
• Reviews current literature relating to nursing practice and PACE care model.

Responsibilities

• Collect assessment data during initial visit and routine re-evaluations to inform the IDT plan of care.
• Work with participant, family, and interdisciplinary team to implement the plan of care.
