Job summary

Alc-Health-Care-Services in Illinois seeks experienced home health care RN s to deliver patient-centered nursing care in facilities and patients' homes.

The role involves regular re-evaluation of patient needs, initiating and revising the plan of care, administering medications, counseling patients and families, coordinating discharge planning, and communicating with physicians to ensure seamless care.

Qualifications

• Minimum of 2 years home health care experience.
• Current RN IL license and CPR certification.
• Strong observation and communication skills; basic math ability.
• Valid driver’s license with insured vehicle.

Responsibilities

• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and revisions as patient status changes.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan with goals and family involvement.
• Initiates preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed.
• Counsels the patient and family with meeting nursing needs.
• Provides health care instructions per assessment and plan of care.
• Identifies discharge planning needs and implements before discharge.
