Job summary

One Home Health is seeking an experienced Home Health RN to join our caring team serving Chicago's North Side and surrounding suburbs. You will manage a home-based caseload, perform skilled visits, and make clinical decisions independently to deliver exceptional patient outcomes.

This part-time, PRN/per diem role offers flexible schedules, competitive pay, and opportunities for professional growth while collaborating with a supportive clinical leadership team.

Qualifications

• Active, unencumbered Illinois RN license.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Minimum of 1-2 years RN experience.
• Strong clinical assessment and critical thinking skills.
• Excellent time management, organizational, and communication abilities.
• Ability to work independently and prioritize a full caseload.

Responsibilities

• Manage a caseload of home health patients and perform skilled nursing visits.
• Admissions, recertifications, resumptions of care, and discharges.
• Complete accurate and timely OASIS assessments and documentation.
• Coordinate care with physicians, therapists, caregivers, and interdisciplinary team members.
• Educate patients and family members on disease management, medications, treatment plans, and home safety.
• Monitor patient conditions and respond appropriately to changes in health status.
• Administer medications, treatments, and skilled nursing interventions as ordered.
• Maintain documentation that meets Medicare, State of Illinois, and agency compliance standards.
• Promote quality patient care and positive health outcomes in the home setting.
