Job summary

CareDiem Home Health in Illinois is seeking compassionate RNs to provide skilled home health nursing to patients in the Bureau County area. You will deliver care under a physician’s order, perform assessments, medication administration, and wound care while traveling to patient homes.

Qualifications

• Illinois RN license required.
• 1+ year nursing experience (home health preferred).
• CPR certification (BLS).
• Reliable transportation, valid driver’s license, and auto insurance.
• Strong clinical judgment, communication, and independence.

Responsibilities

• Deliver skilled nursing care under a physician’s order.
• Complete assessments, medication administration, and wound care.
• Monitor and document patient progress using our electronic charting system (Axxess).
• Educate patients and families on health conditions and care.
• Communicate with physicians and the care team.
• Travel to patient homes.
