Job summary

ALC Home Health is seeking a PRN Physical Therapist Assistant to deliver targeted PT services in patients’ homes. Pay is per standard visit, with flexibility to control your schedule and geographic area of patients.

Responsibilities include performing PT services per physician plans, guiding caregivers, coordinating with the Home Health Director, and contributing to discharge planning. Travel and on-site visits comprise the role, with clinical documentation and occasional education duties.

Qualifications

• Graduate of an accredited physical therapy assistant program with an Associate's degree approved by APTA.
• Current certification or license as required by state law.
• Two years of PTA experience; home health preferred.
• Good verbal and written communication and organization skills.
• CPR certification current.
• Licensed driver with insured vehicle in good working order.
• Independent general liability/workman’s comp coverage/cert.

Responsibilities

• Provide PT services per physician's plan of care; assess status and contribute to total care plan.
• Assess muscle strength, mobility, gait and ROM for rehab potential.
• Direct PT treatment and educate patients and caregivers on appliances.
• Identify needs for other home health services and refer as needed.
• Prepare and submit clinical/progress summaries when goals are met.
• Participate in discharge planning for patients.
• Submit clinical progress summaries per organizational policy.
• Provide PT consultation to families/caregivers as indicated.
• Deliver in-service education to nursing organization personnel as needed.
• Participate in peer consultation and supervise PT assistants per policy.
• Instruct, supervise, and evaluate home health aides every two weeks when therapy is sole skilled service.

This is a PRN position with the following pay rates:

• Standard visits $55.00

The Physical Therapist Assistant employed by ALC Home Health is responsible for completing physical therapy services in adherence to all conditions outlined in the Personnel Service Agreement.

The PRN Physical therapist Assistant position offers:

• Flexibility and Convenience to control your own schedule and geographic area of your patients.
• Administrative support to schedule and track visits utilizing Emergency Medical Response technology with direct communication to Home Health Director.

1. 1. Provides physical therapy services to patients according to a written physician's plan of care. This may include, but will not be limited to:
   • Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.
   • Assesss for muscle strength, mobility, gait, ROM—potential for rehab.
   • Directing physical therapy treatment.
   • Instructing patients and families/caregivers in the use and care of therapeutic appliances.
   • Determining priority needs for physical therapy.
   • Reporting to physician patient’s reaction to treatment or changes in condition.
2. 2. Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
3. 3. May train patient in the use of prosthetic device.
4. 4. Identifies patient and family/caregiver needs for other home health services and refers as necessary.
5. 5. Prepares and submits clinical and progress summaries based on the attainment of goals.

11. 11. Supervises physical therapy assistants according to organization policy and state regulations.
12. 12. When therapy is the only skilled service, instructs, supervises and evaluates home health aide care every two (2) weeks.

Position Qualifications

1. 1. Graduate of an accredited physical therapy assistant program with either an Associate's degree in science or applied science that is approved by American Physical Therapy Association, Inc.
2. 2. Current certification or license as required by state law.
3. 3. Two years of appropriate experience as a physical therapy assistant. Community/home health experience is preferred.
4. 4. Demonstrates good verbal and written communication and organization skills.
5. 5. Possesses and maintains current CPR certification.
6. 6. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
7. 7. Must possess Independent general liability workman’s comp coverage/cert.
