Job summary

Remedy Home Health Care in Ventura County, CA seeks a licensed Physical Therapist (PT) to join our Home Health team. This role offers Per Diem / PRN, part-time or full-time opportunities delivering skilled therapy to geriatric patients in their homes, assisted living, and community settings.

Qualifications

• Licensed Physical Therapist (PT) in California is required.
• CPR certification (AHA approved) required.
• Minimum 1 year of clinical experience; home health preferred.
• Experience with geriatrics, rehab, or community-based care is a plus.

Responsibilities

• Conduct in-home physical therapy evaluations and reassessments.
• Develop and implement personalized rehabilitation care plans.
• Improve strength, balance, mobility, and functional outcomes.
• Provide therapy in home health, assisted living, and community care settings.
• Educate patients and families on fall prevention and safe mobility techniques.
• Document visits using WellSky (Kinnser) EMR.
• Collaborate with interdisciplinary home health teams.
