Job summary

Remedy Home Health Care is seeking a dedicated Physical Therapist to deliver in-home rehabilitation across Ventura County and surrounding areas. The role emphasizes fall prevention, mobility training, and functional independence for geriatric patients.

You will conduct evaluations, develop personalized care plans, and collaborate with nurses, case managers, and physicians in a mobile, community-based setting. A CA PT license and CPR are required.

Qualifications

• Licensed Physical Therapist (PT) - California (Required)
• CPR Certification (AHA approved)
• Minimum 1 year of clinical experience (Home Health preferred)
• Experience in senior care, geriatrics, rehab, or community-based care preferred
• Strong clinical, communication, and patient education skills
• Valid driver’s license, reliable transportation, and auto insurance

Responsibilities

• Conduct in-home physical therapy evaluations and reassessments
• Develop and implement personalized rehabilitation care plans
• Improve strength, balance, mobility, and functional outcomes
• Provide therapy in home health, assisted living, and community care settings
• Educate patients and families on fall prevention and safe mobility techniques
• Document visits using WellSky (Kinnser) EMR
• Collaborate with interdisciplinary home health teams
