Remedy Home Health Care seeks a licensed Physical Therapist (PT) in Ventura County to deliver skilled rehab in home health, assisted living, and community care settings. The role emphasizes autonomy, flexibility, and meaningful, one-on-one care for senior populations in their homes.

Per Diem with part-time or full-time options offers mobility and independence, supported by an interdisciplinary team including nurses and physicians.

Formación

• Licensed Physical Therapist (PT) in California is required.
• CPR certification (AHA approved) is required.
• Minimum 1 year of clinical experience; home health preferred.
• Experience with geriatric and senior care is beneficial.
• Valid driver’s license and reliable transportation.

Responsabilidades

• Conduct in-home physical therapy evaluations and reassessments.
• Develop and implement personalized rehabilitation care plans.
• Improve strength, balance, mobility, and functional outcomes.
• Provide therapy in home health, assisted living, and community settings.
• Educate patients and families on fall prevention and safe mobility.
• Document visits using WellSky EMR and collaborate with the team.
