Job summary

Remedy Home Health Care in Los Angeles County, CA is seeking a licensed Physical Therapist (PT) to provide home health services across Huntington Park, Maywood, South Gate, and surrounding areas. Per Diem / PRN, part-time or full-time opportunities are available, with competitive pay per visit and mileage reimbursement.

You will deliver skilled therapy to geriatric patients in home health, assisted living, and community settings, focusing on fall prevention, mobility, and independence.

Qualifications

Licensed Physical Therapist in California is required.
CPR certification (AHA) is required.
Minimum 1 year of clinical experience; home health preferred.

Responsibilities

Conduct in-home physical therapy evaluations and reassessments.
Develop and implement personalized rehabilitation care plans.
Improve strength, balance, mobility and functional outcomes.
Provide therapy in home health, assisted living and community care settings.
Educate patients and families on fall prevention and safe mobility techniques.
Document visits using WellSky (Kinnser) EMR and collaborate with teams.
