Job summary

West Coast Wound & Skin Care is seeking a skilled Physical Therapist to deliver high-quality, home-based therapy across the Hemet/Temecula region. The PT will evaluate patients in their homes, develop personalized plans of care, and provide manual and therapeutic interventions while documenting progress.

This is a 1099 independent contractor role with a four-day, 10-hour schedule and base pay plus per-visit bonuses.

Qualifications

• Current and valid California Physical Therapist license in good standing.
• Graduate of an accredited Physical Therapy program.
• Strong knowledge of anatomy, physiology, kinesiology, biomechanics, and rehabilitation principles.

Responsibilities

• Travel to patients' homes to provide physical therapy services.
• Perform comprehensive in-home PT evaluations and develop plans of care.
• Provide therapeutic exercises, gait and balance training, and manual therapy as appropriate.
• Educate patients and caregivers on exercises, mobility, safety, and fall prevention.
• Maintain accurate, timely documentation for evaluations, treatments, and discharge summaries.
• Coordinate care with physicians, nurses, and other health professionals.
