Job summary

Beacon Home Health in Spring Hill, TN is seeking a Full Time Home Health Physical Therapist who travels to patient residences to assess and treat mobility, endurance, and pain. You will collaborate on plans of care, monitor progress, and educate patients and families.

Responsibilities include evaluating functional status, assisting physicians with therapy plans, reporting changes, and ensuring optimal function and safety in the home setting.

Qualifications

• Current PT license in the state where services are rendered.
• Current CPR certification.
• Driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.

Responsibilities

• Evaluate the patient's functional status and PT needs.
• Assist the physician in the development of the therapy plan of care.
• Observe and report the patient's response to treatment and any changes in his/her condition to the supervising nurse and/or the physician.
• Treat patients to relieve pain, develop and restore function, and maintain maximum functioning using acceptable standards of practice.
• Instruct and inform participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
