Job summary

Beacon Home Health Services is seeking a Physical Therapist to join our Home Health team serving Kingsport and surrounding communities. The role involves assessing patient needs, developing treatment plans, and delivering interventions to improve mobility and function in a home setting.

Responsibilities include monitoring patient progress, communicating with the care team, and educating patients and families about the plan of care to promote independence and comfort at home.

Qualifications

• Current PT license in the state where services are rendered.
• Current CPR certification, driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.

Responsibilities

• Assess and evaluate patient care needs related to functional status, mobility, endurance, and pain.
• Develop and implement a treatment plan to improve and maintain the patient’s well-being.
• Observe and report patient responses to treatment to the supervising nurse or physician.
• Educate patients, families, and caregivers about the plan of care, functional limitations, and progress toward goals.
