Job summary

Beacon Home Health in Memphis, TN is seeking a Physical Therapist to join our Home Health team. The PT will assess patients, develop and implement therapy plans, and monitor progress across multiple residences in the Memphis area and surrounding communities.

The role requires current PT license in the state, CPR certification, a valid driver's license, vehicle insurance, and reliable transportation to visit patients daily.

Qualifications

• Current PT license in the state where services are rendered.
• Current CPR certification, driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.

Responsibilities

• Assess and evaluate patient care needs related to functional status and mobility.
• Assist physician in developing the therapy plan of care.
• Observe and report patient response to treatment and changes in condition.
• Treat patients to relieve pain, restore function, and maintain max functioning.
• Instruct patient, family and care team about plan of care and progress toward goals.
