Beacon Home Health in Clarksville, TN is seeking a Physical Therapist for Home Health services. The role focuses on evaluating mobility, endurance, and pain to determine and implement an effective plan of care for patients in their homes.

The PT will assess functional status, assist in care planning with physicians, monitor patient progress, and educate patients and families to support recovery and independence in daily activities.

Qualifications

POSITION OVERVIEW

The Physical Therapist is responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on the assessment and evaluation, the Physical Therapist works to help determine a treatment plan, performs interventions aimed at improving and enhancing the patient's well-being, and evaluates the patient's progress.

  • Instruct and inform participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
THE MUST HAVES
  • Current PT license in the state where services are rendered.
  • Current CPR certification, driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
