Job summary

Remedy Home Health Care in Ventura County seeks a licensed Physical Therapist to provide in-home rehab services, focusing on fall prevention, mobility, and independence for senior patients. You will work across Oxnard, Thousand Oaks, Moorpark, Simi Valley, and surrounding areas.

Qualifications

• Licensed Physical Therapist in California (CA) is required.
• Minimum 1 year of clinical experience; home health preferred.
• CPR certification (AHA) is required.
• Experience with geriatrics and senior care is preferred.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Conduct in-home PT evaluations and reassessments.
• Develop and implement personalized rehabilitation plans.
• Improve strength, balance, mobility, and functional outcomes.
