Job summary

Proactive Health in Payson, AZ is seeking a Mobile Patient Onboarding Specialist to visit patients at home, set up health monitoring devices, and guide users through onboarding surveys. You'll work a part-time schedule, starting from your home with routes tailored to minimize drive time.

You will unbox, test, and install devices, provide patient and family instruction with warmth, complete digital intake on a company tablet, and document visit notes in the mobile app.

Qualifications

• Must be 18+ with a high school diploma or equivalent.
• Valid driver’s license and reliable personal vehicle.
• Comfortable navigating tablets and smartphone apps.

Responsibilities

• Travel to scheduled patient visits throughout assigned territory.
• Unbox, test and install cellular biometric monitoring equipment.
• Walk elderly patients and families through device use with warmth.
• Complete a digital intake questionnaire on a company tablet.
• Provide troubleshooting visits and collect equipment after service.
• Document visit notes and patient status in the mobile app.
