Qualifications

• AZ State RN License Required.
• Minimum two years of nursing experience in primary care, behavioral health, community health, urgent care, emergency medicine, hospital nursing, integrated care, or mobile health preferred.
• Ability to work independently in community-based settings and adapt to changing environments.
• Experience serving vulnerable populations (homeless, uninsured, underserved) preferred.

Responsibilities

• Provides nursing services in an integrated healthcare setting.
• Maintains current knowledge and certifications for RN practice.
• Engages patients with treatment planning, education, and support.
• Establishes therapeutic relationships to achieve health goals.
• Coordinates care and follows up on referrals and transitions of care.
• Supports documentation per policy and clinical protocols.
• Participates in community outreach events and mobile unit operations.
• Ensures vaccine storage, infection control, and readiness of mobile unit.
