- AZ State RN License Required.
- Minimum two years of nursing experience in primary care, behavioral health, community health, urgent care, emergency medicine, hospital nursing, integrated care, or mobile health preferred.
- Bilingual in Spanish is a PLUS.
Responsabilidades
- Provides nursing services in an integrated healthcare system.
- Maintains knowledge, skill, and certification for this position.
- Engages patients in treatment, planning, education and support.
- Establishes therapeutic relationships to reach health goals.
- Coordinates care transitions and referrals to support continuity of care.
- Performs community-based nursing care and health screenings in mobile health settings.
- Maintains readiness of Mobile Health Unit and inventory.
