Job summary

Atlas Medical is seeking a Mobile Licensed Behavioral Health Counselor to support a geriatric population within a mobile primary care practice. You will deliver in-home and community-based behavioral health services, coordinating with medical and psychiatric providers to ensure integrated, patient-centered care.

This role requires an active, unrestricted LPC/LCSW or PhD/PsyD licensure, independence, strong organizational skills, and the ability to travel within East Valley and surrounding areas.

Qualifications

• Active, unrestricted LPC, LCSW, or PhD/PsyD licensure.

Responsibilities

• Provide individual therapy and supportive counseling to geriatric patients.
• Collaborate with mobile primary care and psychiatric providers to support integrated care.
• Document visits to meet Medicare and regulatory standards.
• Manage scheduling and travel between patient homes and facilities.
• Support caregivers with education and care planning.
• Maintain professionalism and safety in field settings.
