Responsibilities

About the Role:

Mobile Licensed Behavioral Health Counselor (Geriatric Primary Care) at Atlas Medical. This role partners closely with medical and psychiatric providers to deliver integrated, patient‑centered behavioral health services directly in patients’ homes, assisted living facilities, and other community settings. The counselor will independently manage their own schedule, provide in‑person counseling, and ensure all documentation meets Medicare and regulatory compliance standards.

Location: East Valley, AZ (Mesa / Apache Junction / Gilbert)

Compensation: $75,000+ / FFS DOE

Status: Full‑Time, W2 Employee

Schedule: Monday – Friday, 8 AM – 5 PM

Position Overview

This position is ideal for clinicians who are highly independent, organized, and comfortable working with medically complex older adults. The counselor collaborates with providers to present a whole‑person care model and maintains documentation that meets Medicare guidelines.

Key Responsibilities

Geriatric Behavioral Health Care

• Provide individual therapy and supportive counseling to geriatric patients (primarily Medicare population).
• Address conditions such as depression, anxiety, adjustment disorders, grief, cognitive decline, and chronic illness–related concerns.
• Conduct comprehensive psychosocial and behavioral health assessments.
• Develop and implement individualized, goal‑oriented treatment plans.
• Utilize evidence‑based, age‑appropriate therapeutic interventions.

Integrated Care Collaboration

• Work closely with mobile primary care and psychiatric providers.
• Communicate clinical findings and collaborate on care plans for patients with complex needs.
• Provide caregiver and family education, coping strategies, and care planning support.

Mobile Service Delivery

• Deliver care in patients’ homes, assisted living facilities, and group homes.
• Adapt clinical approach to non‑traditional treatment environments.
• Maintain professionalism, safety awareness, and flexibility in the field.

Scheduling & Productivity

• Independently create, manage, and maintain a consistent weekly schedule.
• Coordinate directly with patients, caregivers, facilities, and referral sources.
• Optimize scheduling routes and volume to maximize productivity and continuity of care.
• Multiple providers will refer patients, ensuring a steady influx of cases.

Documentation & Medicare Compliance

• Complete timely, accurate, and defensible clinical documentation for every visit.
• Ensure documentation supports medical necessity per Medicare guidelines.
• Maintain compliance with CMS regulations, HIPAA requirements, and internal policies.
• Submit documentation promptly to support billing and revenue cycle operations.

Compensation

• Fee‑for‑service model based on completed patient visits and timely submission of compliant documentation.
• No guaranteed salary; compensation is directly tied to productivity and efficiency.

Qualifications

• Active, unrestricted license (required). Licensure must be one of the following: Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or Clinical Psychologist (PhD or PsyD).
• Experience working with geriatric or medically complex populations strongly preferred.
• Knowledge of Medicare documentation standards and medical necessity criteria preferred.
• Experience in home‑based, community‑based, or integrated care settings is a plus.
• Strong clinical, organizational, and independent decision‑making skills.
• Reliable transportation and willingness to travel within the assigned territory.
• Proficiency in EMR/EHR systems.
