Job summary

Atlas Medical in East Valley, AZ is seeking a Mobile Licensed Behavioral Health Counselor (Geriatric Primary Care) to deliver in‑home counseling and coordinate with medical and psychiatric providers. You will manage your own schedule, ensure Medicare-compliant documentation, and work across homes, assisted living facilities, and community settings.

Qualifications

• Active, unrestricted LPC/LCSW/PhD or PsyD license.
• Geriatric or medically complex population experience preferred.
• Home-based/integrated care settings experience is a plus.

Responsibilities

• Provide in-home individual therapy and supportive counseling to geriatric patients.
• Address depression, anxiety, adjustment disorders, grief, cognitive decline, and chronic illness concerns.
• Conduct comprehensive psychosocial and behavioral health assessments.
• Develop and implement individualized, goal-oriented treatment plans.
• Utilize evidence-based, age-appropriate therapeutic interventions.
• Coordinate with mobile primary care and psychiatric providers on care plans.
• Educate caregivers/families and support care planning.
• Deliver care in patients' homes and community settings.
• Maintain timely, compliant clinical documentation for visits.
