Molina Healthcare is seeking a Nurse Practitioner to provide screening and preventive primary care in non-clinical settings, including in-home visits, community sites, and pop-up clinics. You will collaborate with physicians to develop care plans and coordinate services to improve patient outcomes while addressing gaps in care.

The role involves face-to-face visits across diverse populations, potential on-call duties, and travel up to 120 miles per day within the region.

Qualifikationen

• 1 year of NP experience or equivalent.
• Active national certification (AANP/ANCC).
• Active Family Nurse Practitioner license in practicing state.
• DEA license with prescriptive authority.
• BLS certification.
• Valid driver's license and reliable transportation.
• Ability to work across settings with diverse populations.
• Strong communication and self-motivation.
• Responsive in all forms of communication.

Aufgaben

• Provide general medical care and care coordination across populations (adult, women’s health, pediatric, geriatric).
• Perform history and physical exams, preventive assessments, and establish diagnoses.
• Coordinate specialty consultations when needed.
• Order and interpret diagnostic tests judiciously.
• Develop and implement care plans and coordinate post-discharge care.
• Travel locally to patient locations; may require occasional overnight travel.
• Document in EMR and maintain timely records.
