Job summary

Ennoble Care is seeking an experienced Advanced Provider (NP or PA) to deliver primary and palliative care to dialysis patients in DaVita facilities across Union, NJ, on a full-time basis. You will work under the supervision of a physician to care for a diverse population with chronic diseases.

The role requires NJ NP/PA board certification, strong clinical skills, and the ability to operate independently in a supportive, patient-centered environment.

Qualifications

• Board Certified with NJ NP/PA licensure.
• Strong clinical assessment, diagnosis and treatment skills.
• Positive demeanor and strong people skills.
• Ability to problem solve and work independently.
• 1–2 years of primary care with geriatric patients; hospice experience preferred.

Responsibilities

• Provide high quality primary and palliative care in Union, NJ.
• Assess health through examinations, histories, and care planning.
• Communicate effectively with office staff across platforms.
• Deliver patient-centered care to a panel of patients.
• Develop impressions, diagnoses, and treatment plans.
• Consult with patients, families, and caregivers during visits.
• Order diagnostic testing as needed.
• Prescribe medications and medically necessary interventions.
