Job summary

Ennoble Care is seeking an experienced Advanced Practitioner (NP/PA) to provide primary and palliative dialysis care in DaVita facilities around Lehigh, PA, on a full-time basis. You will apply medical techniques under physician supervision to care for patients with chronic diseases.

Board certified with PA/NP credentials in PA, you will leverage strong clinical skills, autonomy, and patient-centered care to support a diverse patient population in a dynamic home/clinic setting.

Qualifications

• Board Certified with a valid NP or PA certification in the State of Pennsylvania.
• 1–2 years of primary care experience with geriatric patients; hospice experience preferred.
• Strong clinical skills with the ability to accurately assess and diagnose; independent practice.

Responsibilities

• Provide high quality primary and palliative care to dialysis patients.
• Perform physical exams, obtain histories, and develop care plans.
• Communicate effectively with office staff and clinical operations across platforms.
• Develop impressions, diagnoses, and treatment plans for a diverse patient panel.
• Foster trust with patients, families, and caregivers during visits.
• Order diagnostic tests as needed and prescribe medications and interventions.
