Job summary

Ennoble Care is seeking an experienced Advanced Provider (NP/PA) to deliver primary care to dialysis patients in the Stroudsburg/Easton, PA area on a full-time basis. You will perform exams, manage care plans, and coordinate with a physician supervisor to ensure comprehensive patient support.

The ideal candidate has PA/NP certification in Pennsylvania and 1–2 years of geriatric primary care, with hospice experience preferred.

Qualifications

• Board Certified, with a valid NP or PA certification in Pennsylvania.
• Strong clinical skills with ability to assess, diagnose and treat.
• Positive demeanor and strong people skills.
• Ability to problem solve and work independently.
• 1–2 years of primary care experience with geriatric patients; hospice experience preferred.

Responsibilities

• Provide primary and palliative care to dialysis patients in Stroudsburg/Easton, PA region.
• Perform physical examinations and update medical histories to plan care.
• Communicate effectively with office staff across platforms.
• Deliver patient-centered care to a panel of patients.
• Develop impressions, diagnoses and treatment plans.
• Consult with patients, families and caregivers during visits.
• Order diagnostic testing as medically necessary; prescribe medications and interventions.
