Job summary

Concierge Home Care is seeking a Mobile Dermatology Nurse Practitioner to provide patient-centered dermatology services in the community and patients’ homes in Jacksonville, FL. The role offers clinical autonomy with a mix of in-person and telehealth visits, and collaboration with a growing team.

The ideal candidate holds a Master's NP degree, board certification (FNP/AGNP), dermatology experience or interest, and is able to travel within the service area.

Qualifications

• Master's degree from an accredited Nurse Practitioner program.
• National board certification such as FNP or AGNP.
• Dermatology clinical experience or interest in dermatologic care.
• Current DEA registration or ability to obtain, if required.

Responsibilities

• Evaluate patients and manage dermatologic conditions in home and telehealth settings.
• Develop individualized treatment plans and coordinate follow-up care.
• Prescribe medications and monitor treatment effectiveness.
• Coordinate care with primary care providers and dermatologists.
