Job summary

Community Health Resources is seeking a Crisis Therapist to join a fast-paced mobile crisis team in Middletown, CT. You will conduct mobile assessments, deliver crisis interventions, and ensure follow-up with youth and families.

The role collaborates with schools, hospitals, DCF, law enforcement, and community providers, while upholding strong clinical standards. Opportunities for growth and leadership are included.

Qualifications

• Master’s Degree or higher in related field.
• Experience working with trauma, mental illness and/or SUD preferred.
• Licensure/Certification: LMSW, LPC-A, LMFT-A required; other licenses may be required; valid driver’s license.

Responsibilities

• Provide assessment and treatment interventions consistent with the program model.
• Conduct clinical and risk assessments.
• Collaborate with individuals to create individualized, person-centered treatment plans.
• Demonstrate competency in age-related or specialty issues and developmental needs.
• Develop discharge plans utilizing natural supports and community linkage.
• Maintain collaborative relationships with family members and other service providers.
• Provide crisis assessment and intervention to monitor crisis status.
• May provide care in office, home, residential, or community settings as required.
• Maintain client and staff records per agency policy.
• Provide coverage for team members during absences.
• Participate in weekly supervision, consultations and meetings.
