Job summary

Community Health Resources seeks a Crisis Therapist for the Mobile Crisis Intervention Services in Middletown, CT. This per diem position balances direct crisis work with collaboration across agencies to support youth and families in crisis.

You will conduct mobile assessments, deliver crisis interventions, and ensure follow-up linkage to ongoing services, coordinating with schools, hospitals, DCF, law enforcement, and community providers.

Qualifications

• Master’s degree or higher in related field.
• Experience with trauma, mental illness, and/or substance use disorders preferred.
• Licensure/Certification: LMSW, LPC-A, LMFT-A required; LCSW/LMFT/LPC or Licensed Psychologist preferred.
• Valid Driver’s License for programs requiring transportation.

Responsibilities

• Provides assessment and treatment interventions aligned with the program model.
• Conducts clinical and risk assessments.
• Collaborates to create individualized, person-centered treatment plans.
• Develops discharge plans with natural supports and family involvement.
• Maintains client and staff records per agency policy.
• Participates in weekly supervision and team meetings.
