Canvas Health seeks a Mobile Crisis Practitioner to join our team in Chaska, MN. The role focuses on triage, crisis assessment, and intervention across in-home, clinic, and telehealth settings, collaborating with the mobile crisis response team to help individuals in crisis.

The position supports a mission-driven nonprofit delivering high-quality behavioral health and crisis services, with a commitment to equity and community impact in Minnesota.

Qualifications

• Bachelor's degree in Behavioral Sciences or higher.
• 2,000+ hours delivering supervised services to clients with mental illness, substance use disorders, or emotional disturbance.
• Alternate: currently serving a federally recognized tribe and qualifies as a vendor of medical care.

Responsibilities

• Answer phones to triage mobile crisis needs and determine service level.
• Provide diagnostic and crisis assessment, crisis intervention, and related therapeutic services.
• Deliver services in-home, office-based, and via telehealth as part of a mobile crisis response team.

$5,000 Retention Bonus!!

The candidate selected for this position may be eligible for a retention bonus of up to $5,000. An initial sum of $2,500 will be paid after the completion of six months in this role, with the balance disbursed after twelve months of service. To qualify for the full bonus, the individual must maintain employment in this position for at least twelve months. The bonus is non-transferable to other roles.

Salary

This area has been designated as a Recruitment Desert. Employees who primarily work in a Recruitment Desert will receive a15% differential pay in addition to their base salary (which is already included in the salary range).

Practitioner

Base Salary: $54,549.47 - $63,162.54/annually

Base salary + Recruitment Desert Differential: $62,731.89- 72,636.92/annually

The expected compensation range for this position represents a good faith estimate for this position and the specific compensation offered to a candidate may vary based on factors including, but not limited to, the candidate's relevant knowledge, training, skills, work location, experience, and/or FTE.

Position Summary

This position is responsible for answering phones to triage and determine the need for mobile crisis response, providing diagnostic and crisis assessment, crisis intervention and additional therapeutic services as indicated, as a member of a mobile crisis response team. Services are provided in-home (Mobile Crisis Response), office-based, and telehealth.

Location

Chaska, MN

Schedule

40 hours per week, Monday through Friday, typical hours are between the hours of 8am to 5pm

Minimum Qualifications

Practitioner

• Possess a bachelor's degree or higher in behavioral sciences.

AND

• Completed 2,000 or more hours of delivering supervised services to clients with mental illness, substance use disorder, or emotional disturbance

OR

• Currently serving a federally recognized tribe and meets the definition of "vendor of medical care."

Preferred Qualification

• Experience working with and/or delivering services to diverse populations
• Experience delivering either in-home or crisis services
• Experience with Electronic Health Record systems
