Coleman Health Services is seeking a Crisis Counselor to deliver crisis intervention, assessment, safety planning, and brief counseling across Highland, Ross, Pickaway, Fayette, and Pike Counties in Ohio, based out of Chillicothe. The role involves mobile crisis response, collaboration with a team, and follow-up care for adults and families in crisis.

Responsibilities include crisis assessments, EMS/Police referrals, and data tracking to meet state requirements.

Formación

• Master's degree in social work, Counseling, or a related behavioral health field.
• Current Ohio licensure (LSW/MSW, LISW, LPC, LPCC).
• Eligible to obtain or currently have an NPI number and Ohio Medicaid provider number.
• Valid driving record.

Responsabilidades

• Foster crisis intervention using triage, engagement, assessment and trauma-informed care.
• Perform crisis assessments and risk screening for EMS/Police referrals.
• Travel to provide mobile crisis assessment/intervention across multiple counties in OH.
• Coordinate with multidisciplinary teams to connect clients with inpatient/outpatient services.
• Determine need for higher level of care and coordinate hospital/community referrals.
• Track data for mental health board and state requirements.
• Complete training to become a community health officer.
