The Link & Option Center, Inc. is a Joint Commission–accredited outpatient behavioral health clinic serving the Southland region with locations in South Holland and Hazel Crest, IL.

The CST Peer Support Specialist provides direct, community-based mental health and recovery support under QMHP supervision and helps clients develop independent living skills and access services. The role focuses on addressing Social Determinants of Health, collaborating with a treatment team, and delivering services

Qualifications

• Provide services under QMHP supervision and meet the minimum qualifications: high school diploma, 21+ years old, ability to deliver rehabilitative services to adults or children, work within an agency structure, accept supervision, and engage with clients and community.

Responsibilities

• Provide 60% of face-to-face rehabilitative services in community settings
• Be available 24/7 to support Mobile Crisis Response
• Develop independent living skills, medication self-management, social interaction
• Support clients with mental health symptoms and co-occurring substance use disorders
• Monitor changes in symptoms and functioning, report progress
• Assist with addressing SDOH needs and care planning
• Coordinate with multidisciplinary teams for integrated care
• Document interactions within 24 hours in EHR
• Travel regularly to meet individuals in the service area
• Flexible scheduling including evenings or weekends
