• Completes CHW training/certification within 6 months of employment.

Responsibilities

• Serve as a liaison between the healthcare system, community organizations and patients, connecting them with local resources.
• Conduct screenings for health-related social needs and document in Epic, with follow-up on health and wellbeing.
• Engage with patients proactively, showing initiative and leadership in the clinic setting.
• Bilingual in Spanish is preferred but not required.
