Medical Teams International is seeking a Community Health Worker for our mobile medical unit in Hermiston, Oregon. The role provides outreach, health education, care coordination, and navigation support to help underserved populations access services.

You will work with partners, assist with health plan enrollment, connect patients to primary care, maintain documentation in the EHR, and travel regionally as needed. Bilingual English/Spanish preferred. HIPAA compliance required.

Qualificações

• High school diploma or GED is required.
• Active OHA Trained Assister certification or equivalent required.
• Valid driver's license required.
• BLS certification from the American Heart Association.

Responsabilidades

• Identify and address barriers to care; assist with Health Plan enrollment and insurance navigation.
• Connect patients to primary care and confirm attendance at initial appointments.
• Provide culturally responsive health education on prevention, vaccines, and chronic disease management.
• Maintain up-to-date resource lists and refer to community services and housing, nutrition, and behavioral health providers.
• Collaborate with case managers and clinical teams; document in the electronic health record; ensure HIPAA compliance.
