Job summary
Your Health Organization in Georgetown area seeks a dedicated Community Health Worker (CHW) to link patients with care, services, and community resources. The role includes home and ALF visits, delivering medications and DME, and coordinating with providers to ensure timely access to care.
The CHW must have reliable transportation as daily travel is required. This is a full-time, Monday–Friday position (8:00 AM–5:00 PM) with a competitive compensation package and benefits.
Qualifications
- Certified Medical Assistant, Paramedic, or Licensed Nurse in good standing.
- Phlebotomy experience preferred; willingness to train if not experienced.
- Public health or home health background preferred.
- Experience with DME delivery or logistics operations preferred.
- Proficient with remote patient monitoring and telemedicine platforms.
Responsibilities
- Coordinate and deliver home and facility visits to support patient care.
- Deliver medications and DME to patients’ homes and ALFs; review instructions.
- Facilitate telehealth visits with pharmacists, specialists and providers.
- Set up RPM equipment and provide ongoing patient education and troubleshooting.
- Assist with venipuncture collection and proper specimen handling.
