Job summary

LifeSkills and Pennyroyal Center in Bowling Green, KY is seeking a Case Manager to provide comprehensive case management for individuals with SMI, SUD, or SED. The role focuses on resource linkage, care coordination, and client advocacy to support recovery in clinic and community settings.

Travel up to 50% and a 1st shift schedule are part of the position. Candidates should have a 4-year degree in a behavioral science and reliable transportation.

Qualifications

• Requires a Bachelor of Arts or Science degree in a behavioral science field.
• Must have reliable transportation with driver’s license and auto insurance.
• Strong written and verbal communication skills.

Responsibilities

• Assess client and family strengths and needs.
• Link client and/or family to appropriate community resources.
• Assist client/family with advocacy efforts with other community services.
• Maintain contact with client/family (4+ contacts per month, including 2 face-to-face).
• Collaborate with LifeSkills staff and external providers to support recovery.
• Provide services in the clinic and community (homes, school, courts, DCBS, etc.).
• Assist individuals to engage in activities for meaningful roles in the community.
• Use personal vehicle to transport individuals (must have license and insurance).
• Provide billable targeted case management services to 23–25 clients per month.
• Attend supervision and complete timely documentation.

Education
