Chartiers Center in Kentucky seeks an Intensive Mobile Case Manager to coordinate behavioral health, medical social services, and community supports for program participants. You will assess needs, develop individualized plans, meet participants regularly, maintain compliant documentation, and participate in rotating on-call duties to support after hours.

• Bachelor’s degree in human services or related fields with mental health experience required.
• Associate's degree in human services or related fields with mental health experience required.
• Valid Driver’s License is required.

Responsibilities

• Coordinate and manage the delivery of behavioral health, medical social service and community support services.
• Assess participant needs and develop individualized, person-centered service plans.
• Meet with participants regularly to provide ongoing case management and monitor progress.
• Maintain accurate, timely documentation in compliance with requirements.
• Participate in rotating on-call responsibilities to support after-hours for Janus program participants.
