Job summary

CAYA Collaborative Care in Kentucky is seeking a Targeted Case Manager to provide comprehensive case management for individuals with severe mental illness, substance use disorders, or serious emotional disturbance in clinic and community settings.

You will assess needs, link to services, advocate for clients, coordinate care across physical and mental health, maintain regular contact (4 visits/month, 2 face-to-face), document diligently, and use your vehicle to transport clients as required,

Qualifications

• Bachelor or higher in a behavioral science field required.
• One year post-graduate experience with mental health/SUD/SED.
• Master’s degree may substitute for experience per state regulations.
• Must have reliable transportation with driver’s license and auto insurance.

Responsibilities

• Assess client and family strengths and needs.
• Link client and/or family to appropriate community resources.
• Advocate for client with other community services.
• Maintain contact with client/family (minimum 4 contacts per month, including 2 face-to-face).
• Collaborate with Lifeskills staff and external providers to aid recovery.
• Provide services in clinic and community settings.
• Transport clients using personal vehicle when required.
• Provide billable targeted case management for 23–25 clients per month when possible.
• Attend supervision sessions and document services timely.
• Perform other duties as assigned.
