Job summary

Integrea Community Mental Health System in Alabama is seeking a dedicated Case Manager to support individuals with Serious Mental Illnesses, coordinating care and advocating for essential services.

You will conduct assessments, develop service plans, facilitate access to housing, employment, and community resources, and maintain compliant documentation within a multidisciplinary team.

Qualifications

• Experience providing case management to adults with Serious Mental Illness (SMI).
• Ability to coordinate care across services and settings.
• Strong documentation and regulatory compliance skills.

Responsibilities

• Provide direct case management services to individuals with SMI, including assessment, service planning, referral, linkage, monitoring, and advocacy.
• Develop and implement individualized service plans in collaboration with consumers, families, and interdisciplinary teams.
• Assist clients in accessing mental health, medical, housing, employment, educational, and social support services.
• Conduct home visits, community outreach, and transport clients to appointments and community resources as needed.
• Monitor consumer progress and update service plans accordingly.
• Maintain accurate and timely documentation in compliance with agency and regulatory standards.
• Participate in multidisciplinary team meetings, supervision, and case reviews.
• Ensure coordination of care across service systems to support holistic client wellness.
