Job summary

Prairie View, Inc. seeks a Care Coordinator for our Mobile Unit, based in Newton, KS. You will travel with a multidisciplinary team to assess needs, develop treatment plans, manage crises, and connect clients to community resources.

Responsibilities include direct client care, case management, documentation, and coordination with local agencies. State of Kansas PCCM certification (or willingness to obtain) is required, with CEUs as applicable.

Qualifications

• Minimum two years working with adults who have substance use disorder and/or SMI.
• Excellent verbal, written, and listening communication skills.
• Proficient at basic computer and keyboarding skills.

Responsibilities

• Direct Client Care & Case Management for adults and adolescents in recovery.
• Develop, monitor, and adjust treatment plans (every 90 days) with clinical staff.
• Coordinate with agencies to ensure service access and community resources.
• Maintain clinical documentation per state licensing and agency standards.

Skills

Two years with adults SUD/SMI

Excellent communication skills

Basic computer/keyboarding

Education

Kansas PCCM certification (or willing to obtain)
