Job summary

Vibrant Health is seeking an Integrated Care Coordinator in Kansas City to support a team-based care model within a PCMH at an FQHC. You will coordinate visits, review care plans, and track patient metrics using Azara DRVS to identify care gaps and follow up with patients.

The role requires collaboration with Medical Assistants, Nurses, and providers, strong communication skills, and the ability to connect patients with community resources to improve access and outcomes.

Qualifications

• Experience in a team-based care/PCMH/FQHC setting.
• Proficiency with EHR and patient management systems.
• Strong interpersonal and communication skills.

Responsibilities

• Coordinate care plans and ensure documentation before visits.
• Track patient metrics and risk stratify using Azara DRVS.
• Facilitate care team huddles and post-visit follow-ups.
• Identify and address barriers to care (transport, scheduling, literacy).
• Document all patient interactions in the EHR accurately.
• Connect patients with community resources to improve access.
