Job summary

Prairie View in Newton, KS is seeking a Care Coordinator for our Mobile Unit. You will travel with a multidisciplinary team to provide direct support to adults and adolescents across the tri-county area, assess needs, develop treatment plans, manage crises, and connect clients to community resources for lasting recovery.

This full-time role requires a minimum of two years of experience with substance use disorder and/or mental illness, Kansas PCCM certification or willingness to obtain it, and

Qualifications

Two years of experience working with adults who have substance use disorder and/or severe mental illness.
Must hold or be willing to obtain the Kansas PCCM certification including CEUs.
Excellent verbal, written, and listening communication skills.
Proficient with basic computer and keyboarding.

Responsibilities

Provide direct client care coordination and crisis intervention for adults and adolescents.
Develop and monitor treatment plans every 90 days with clinical oversight.
Assess client needs to promote autonomy and recovery.
Educate clients on service choices and coping skills.
Build relationships with local agencies to connect clients to resources.
Maintain documentation in compliance with licensing standards.
