Job summary

UPMC Community Care Behavioral Health is seeking a full-time Mobile Professional Care Manager to support the Care Management Department in Mercer & Venango Counties. The Mobile Professional Care Manager will travel throughout Mercer and Venango Counties and work flexible hours, with a portion of time spent remotely.

The role conducts assessments, coordinates care across physicians, and facilitates communication between health plan members and community-based services.

Qualifications

• Pennsylvania licensure in health or human services field and master's degree OR licensed RN (BSN preferred)
• Par Tam?slike Paramedic/EMT with 6-8 years of experience
• Three years of experience in behavioral, clinical, utilization management, home care, discharge planning, and/or case management
• Prior experience in behavioral health or SUD highly preferred

Responsibilities

• Conducts face-to-face member assessments in the community, home, or facility.
• Coordinate with member's physicians to ensure follow-up and coordination of care.
• Collaborates with providers for initial assessment, treatment planning, and aftercare planning.
• Facilitates linkages between primary care and behavioral health providers and social services.
• Ensures documentation and case management are within established timeframes.
• Responds to complex and crisis calls and coordinates care across the continuum.
• Identifies barriers to care and develops integrated plans with the member, family, and providers.

Education

Master's degree in health or human services

Licensed RN (BSN preferred)
