Job summary

Strategic Consulting Partners (SCP) seeks a Care Manager Associate to support UPMC Health Plan members in the community, coordinating care after hospital or skilled nursing stays and addressing medical, behavioral health, or social determinants.

Hybrid role with travel 50-75% across Allegheny and Westmoreland Counties; after orientation, remote work possible when not in the community; collaborates with multidisciplinary teams and documents care activities in the tracking system.

Qualifications

• Bachelor’s degree in social work or associate degree in another health/human services field.
• No license required.
• Managed care experience preferred.
• Ability to interact professionally with health care professionals.
• Excellent verbal and written communication; knowledge of community resources preferred.
• Value for and ability to deliver excellent customer service.

Responsibilities

• Review Health Plan data and identify gaps in care.
• Assist with transportation, scheduling appointments, and tracking utilization.
• Refer members to case management, health management, or lifestyle programs.
• Assist members with non-medical needs affecting health and access to care.
• Develop an individualized plan of care with the member’s care team.
• Document activities in the care management tracking system per Health Plan standards.
• Advocate for services access and patient safety and rights.
• Maintain knowledge of health benefits and plan policies.
• Interface with community-based resources and supportive services.
• Perform other duties as assigned.
