UPMC Community Care Behavioral Health in Camp Hill offers a hybrid role for a Licensed Professional Care Manager. You will travel in the community and work mostly remotely with occasional in-office presence, with mileage reimbursed at IRS rates.

You will coordinate care for members at risk with complex needs, handle pre-certifications, discharges, and service authorization, and lead clinical initiatives within the care management team to improve outcomes.

Formación

• Active license as a behavioral health professional.
• Experience in care management and coordination for at-risk populations.
• Strong communication and collaboration skills.

Responsabilidades

• Makes authorization determinations for medically necessary services within licensure.
• Coordinates care with primary care and behavioral health providers integral to the member's life.
• Monitors treatment outcomes and adjusts plans to provide optimal community-based care.
• Manages designated client caseload and coordinates information for reviews and reporting.
• Educates members, providers, and families on benefits and coverage.
• Provides clinical reviews, service authorization, and care coordination for assigned members.
• Participates in CQI activities and professional development.
