Job summary

UPMC Community Care Behavioral Health seeks a Mobile Professional Care Manager for full-time work across Bedford and Blair Counties with travel and community-based visits. The role blends travel in the community with remote work from home at times.

Responsibilities include conducting member assessments, coordinating care among physicians, behavioral health, and social services, and ensuring timely documentation and care plans. Licensure in PA and clinical experience are required.

Qualifications

• Pennsylvania licensure in health or human services field and master's degree OR RN (BSN preferred) OR Paramedic/EMT with 6-8 years of experience.
• Three years of behavioral, clinical, utilization management, home care, discharge planning, or case management experience.
• Five years of community-based case management and behavioral health experience preferred.
• Strong communication skills and detail-oriented with good organizational abilities.

Responsibilities

• Conducts face-to-face member assessments in community, home, or facility settings.
• Coordinate with physicians for follow-up and care coordination.
• Facilitate communication among providers, social services, and health plans.
• Manage documentation within established timeframes and participate in case conferences.
• Identify barriers to care and develop integrated care plans with members and families.
