Responsibilities

This is acommunity-based, mobile positionwhereyou’ll meet people where they are emotionally, physically, and withintheircommunities. You’ll work alongside individuals, families, providers, andmultidisciplinary teams to ensure each person receives coordinated,person-centered support.

Pay range begins at $52,000

What You’ll Do

• Buildmeaningful relationships by meetingindividuals where they are and creating trusting, respectful, andsupportive connections.
• Coordinateindividualized care by partnering withindividuals, families, natural supports, service providers, and recoveryteams to develop, implement, and update personalized recovery plans.
• Connectpeople to resources by identifying communityservices and completing and sending referrals for needed supports.
• Collaboratewith providers including therapists,healthcare professionals, case managers, and other community partners topromote coordinated, high-quality care.
• Supportrecovery goals by monitoring progress towardself-identified goals and providing supportive coaching, psychosocialrehabilitation, and symptom-management support.
• Stayconnected through regular in-personvisits, phone calls, and follow-ups.
• Helpremove barriers by ensuring individuals haveaccess to transportation for program-related appointments.
• Advocatefor individuals by helping them access timelyand appropriate services while empowering them to recognize and build ontheir strengths.
• Supportindividuals through challenges by respondingto crises and setbacks with empathy, professionalism, andatrauma-informed approach.
• Documentservices accurately by maintaining timely andthorough documentation in accordance with agency, county, regulatory, andfunding-source requirements.
• Staycurrent and compliant by completing all requiredcounty and agency trainings and following agency policies, procedures,Code of Conduct, and Core Values.

Required Qualifications

• Bachelor’sdegree inSocial Work or a related field.
• Oneyear of experience working with individuals experiencingmentalhealth, substance use, and/or co-occurring needsis preferred.
• Abilityto meet all WCS and contractor requirements, including criminal andcaregiver background checks, driver’s record review, and drug screening.
• Validdriver’s license, reliable automobile, and sufficient insurance to meetagency requirements.

Preferred Qualifications

• Master’sdegree inSocial Work or a related field.
• Experienceworking withMilwaukee County Behavioral Health Services.
